I was sitting in a hardware store parking lot with a bag of drywall anchors on the passenger seat when my phone buzzed. Patient portal. Two new results. Hemoglobin flagged in red at 12.6. Ferritin flagged in red at 11.
Twenty years ago I would have learned those numbers in a phone call from a nurse three days later, if at all. Instead I had them in my hand roughly ninety minutes after the blood left my arm. That is an electronic health record doing exactly what it was built to do.
I then spent about four seconds thinking about it, decided I already knew what it meant, and drove home to install a shelf. It took fourteen months, a worse second set of numbers, a referral, two removed polyps, and a bill I did not expect before I understood what those red flags had been telling me.
I want to write about what electronic health records actually do for patients, because I have now experienced most of the benefits people list, and I have also experienced the gap between a benefit existing and a benefit reaching you. Every advantage below is real. Not one of them is automatic.
Your Results Reach You Directly, Which Is Both the Benefit and the Risk
The most immediately visible thing an EHR does for a patient is put their own information in their hands. Under federal information sharing rules, results now generally flow to the portal as soon as they are finalized rather than sitting until a clinician reviews them.
My first instinct, after everything that followed, was to resent this. I was wrong. Research summarized by the Office of the National Coordinator found that more than 95 percent of surveyed patients wanted to keep receiving immediately released results. That figure held even among patients whose results came back abnormal. People want their information. The same research also found those patients had unmet needs in understanding what they were looking at.
That gap is precisely where I fell in. The failure was not that I got the result early. It was that a number arrived with no context and I supplied the context myself, badly, in a parking lot.
I had explanations ready. I had given blood in March. I had been running more that spring than in a decade. I had drifted away from red meat sometime the previous fall. Any one of those was plausible. Stacked together they felt like an answer.
A plausible explanation is not a diagnosis. That is the single most useful sentence I took out of this, and no software can install it for you.
What I do now with early results: I read them, I write down questions, and I conclude nothing. Getting a result early is a head start on a conversation, not a replacement for one.
Record Holds a Trend Line You Cannot Hold in Your Head.
The benefit people usually name first is the complete medical history, and it is genuinely the foundation of everything else. But the version that mattered in my case was narrower and more interesting: an EHR does not just store results; it stacks them.
Ferritin measures the protein that stores iron in your body, which makes it a reasonable proxy for iron reserves. MedlinePlus explains what a ferritin blood test actually measures, including the important caveat that an abnormal number does not automatically mean a condition requiring treatment. Reference ranges also vary between laboratories.
Mine was 11 for an adult man; that is not borderline.
Here is what I did not grasp: iron deficiency anemia is not really a diagnosis. It is a finding that raises a question, and the question is where the iron is going. Not enough coming in, not being absorbed, or being lost somewhere. I assumed the first, because the first was flattering and required nothing of me except a supplement.
My physician did what a reasonable physician does with a mild first result. Portal message: start iron, recheck the blood count in three months.
I did the first half. The iron worked, which is exactly the problem. Within five weeks I was not winded on the stairs, a thing I had not consciously registered as a problem until it stopped. Treating a symptom successfully removes your motivation to find its cause. The bottle ran out at month four, and I did not buy another one, because I felt fine.
Fourteen months later, at a work physical, the numbers were worse than before I had ever taken a tablet.
But when a gastroenterologist finally opened my chart, all of it was sitting there in sequence. Two data points fourteen months apart, trending the wrong direction, in a record I had personally lived through and completely failed to read as a line. The chart understood my situation better than I did, and it had understood it the entire time.
Handoff That Used to Take Weeks
When I was referred, I brought nothing. No envelope of films, no printed results, no attempt to recite my own history from memory in a waiting room.
This is the benefit that gets undersold because it is invisible when it works. The specialist had my prior labs, my medication list, my primary care notes, and the actual values from both blood draws before I sat down. Nobody re-ran a blood count that had already been run. Nobody asked me to estimate when something happened.
Reduced duplicate testing is usually framed as a cost saving, and it is. From the patient’s chair it reads as something else: fewer needles, fewer trips, and a shorter distance between noticing a problem and doing something about it.
It also surfaced something I had forgotten. A reaction to an antibiotic, documented at a different health system more than a decade earlier, appeared on my chart during pre-procedure review. I would not have thought to mention it. I had genuinely stopped remembering it.
Alerts Are Real, and So Are Their Limits
Safety alerts are the benefit most often cited and the one I would ask you to hold most loosely.
The mechanism is sound. Systems flag drug interactions, documented allergies, doses outside expected ranges, and overdue screenings. Mine caught a supplement on my medication list before the procedure that the nurse wanted to discuss. That is a small thing that could have been larger.
But I went looking at the evidence afterward, and the picture is more complicated than the marketing. The AHRQ Patient Safety Network’s primer on alert fatigue describes clinicians in one Veterans Affairs primary care study receiving more than a hundred alerts per day, and notes that clinicians override the vast majority of order entry warnings, including critical ones. The reason is not carelessness. It is that most alerts are clinically inconsequential, so ignoring them is usually correct, and the habit of ignoring them does not distinguish between the trivial and the serious.
An alert system that fires constantly is, functionally, an alert system that is off. This does not mean the alerts are worthless. It means the useful version is the specific, well-tuned one, and that a patient should not assume the software is catching everything a human might miss.
Where My Record Did Not Save Me
Preventive reminders and follow-up tracking are, on paper, one of the strongest arguments for electronic records. The system can flag overdue screenings, prompt recalls, and hold the thread on a plan across months.
Mine did not fire, or if it did, it did not reach me.
I was 46 when I read those first results. The US Preventive Services Task Force recommends colorectal cancer screening beginning at age 45 for adults at average risk. I had never been screened. Nobody raised it, and I did not ask.
More pointed: a “recheck in three months” instruction is only a plan if someone owns it. Mine lived in a portal message. There was no recall, no reminder at week ten, no standing order waiting for me. The system had recorded an intention. Recording an intention is not the same as pursuing one, and this is the ordinary, undramatic way these systems fail, not with a wrong number. With a thread nobody picked back up.
The clinical reasoning behind what happened next was not mysterious once someone explained it. The American Gastroenterological Association’s clinical practice guidelines on the gastrointestinal evaluation of iron deficiency anemia recommend bidirectional endoscopy for men and postmenopausal women with iron deficiency anemia, since unexplained iron loss in those groups warrants examining the gastrointestinal tract. Those guidelines also recommend a ferritin cutoff of 45 nanograms per milliliter rather than 15 for diagnosing iron deficiency in anemic patients, which I found clarifying, because it meant “technically within some range” was never the reassurance I had treated it as.
The colonoscopy found two adenomas, the larger about 12 millimeters. Both were removed during the procedure. Pathology showed low-grade dysplasia, and I was placed on a shorter surveillance interval than the standard ten years. Intervals are decided case by case.
That is not a dramatic ending, and I am glad. Removing an adenoma is not treating cancer. It is removing something that might, across years, have become one.
Sentence That Never Made It Into the Note
There is a detail I have thought about more than any other.
During that first appointment, while my physician was turned toward the screen, entering something, I mentioned that a couple of months earlier the toilet water had looked strange once or twice. I said it the way you mention weather, to the side of someone’s head, signaling by my own tone that it did not matter.
It did not make it into the note. I know because I later read the note.
I am not blaming her. Documenting in real time while conducting a visit is genuinely hard, and an offhand comment from a patient who is himself dismissing it is exactly what falls through. But that sentence sits near the top of the CDC’s list of symptoms worth raising with a doctor about colorectal cancer, a list that also includes low iron levels in the blood. I had mentioned two items on that list, one without meaning to, and neither of us connected them.
This is where I became interested in the documentation problem rather than just irritated by it. Ambient documentation tools, often described as an AI Medical Scribe, listen to the visit and draft the clinical note so the clinician can face the patient instead of the keyboard. A multicenter study in JAMA Network Open reported clinician burnout falling from 51.9 percent to 38.8 percent after thirty days of ambient AI scribe use, alongside improvements in cognitive task load and attention to patients.
I want to be careful with that finding, because it is being oversold in places. It was a quality improvement study with self-reported outcomes and no control group. It measured how clinicians felt, not whether patients were diagnosed more accurately. Nobody has demonstrated that an AI scribe captures the sentence I muttered sideways, and a fluent note is not automatically a correct one. The draft still needs review.
But the underlying problem is real, and I walked straight into it. Attention is finite. Every minute spent transcribing is a minute not spent noticing.
Reading Your Own Chart Is the Most Underused Benefit
The practical response costs nothing, and almost nobody does it.
After every visit now, I open the visit summary and read it. Not skim. Twice I have found something missing or wrong, and both times a short portal message fixed it within a day.
Your chart is not a transcript. It is a compressed, interpreted account written under time pressure, and it is the version every future clinician will actually see. If the important thing is not in there, functionally it did not happen.
Two habits followed from that. Say the important thing when you have eye contact, not while someone is typing. And if you are unsure it landed, ask: “Is that going in the note?” It reads as confrontational written down. Out loud, nobody has ever reacted badly.
Benefit Hiding in the Billing Office
I had absorbed somewhere that colonoscopies are free now. Roughly true for average-risk screening under most plans. Not true for mine, because mine was not a screening.
The distinction turns on why the procedure was ordered. A colonoscopy performed on someone with no symptoms, on the preventive schedule, is generally classified as screening. One performed to investigate an abnormal result, which is what mine was, is generally classified as diagnostic, and diagnostic procedures typically run through your deductible and coinsurance: same scope, same room, same physician, different classification, very different number at the bottom.
That charge was legitimate and correctly applied. It was also entirely predictable, and I would have predicted it by asking one question at scheduling: is this being ordered as screening or diagnostic, and what does my plan do with each?
Then a separate bill arrived eleven weeks later from a reference laboratory I had never heard of, for pathology on the removed tissue. That one I disputed, because the codes did not match what I had been told, and it took three calls to resolve.
Which sent me down a rabbit hole about how any of this works upstream. When an EHR is properly connected to the billing workflow, the diagnosis codes, the order, and the claim all describe the same event, and the statement you receive is something you can reconcile. What I Would Ask Before Choosing a Practice Now
I used to choose providers the way I choose restaurants. Ratings, parking, whether the office looked nice.
What actually mattered was whether the practice had a working process for closing the loop on an abnormal result. Mine did not fail dramatically. It failed quietly, by assuming a patient told to come back in three months would come back in three months.
So the questions changed:
Who reads results, and what happens when one is abnormal?
Ask the front desk. A practice with a real process will describe a nurse, a queue, a recall list. One without will say the doctor reviews everything.
Which lab is running this, and is it in network?
The lab is frequently a separate business, billed separately. This is where a surprising share of unexpected charges originate.
Is this order screening or diagnostic?
Ten seconds, asked before rather than after.
What is the follow-up specifically, and who owns it?
“Recheck in three months” is not a plan. “The reminder goes out at week ten, and the order is already in the system” is a plan.
What the Record Can and Cannot Do
The honest summary is that electronic health records solved the problems of the paper chart almost completely. My history follows me. My specialist had everything. My results reached me the same day. A decade-old allergy surfaced without my remembering it. Nobody re-ran a test that had already been run.
What they did not solve is attention. A record is a very good memory attached to a system that still depends on someone deciding to look, and on you deciding to ask.
Fourteen months is not a catastrophe. In my case it was a delay rather than a disaster, and I am aware some of that was luck. But it was fourteen months spent believing I had handled something, inside a system that had already recorded, accurately and in order, that I had not.
The record was never the weak link. I was. The useful conclusion is not that the technology failed but that a benefit only becomes a benefit at the moment somebody uses it.
